Rejuvenated Legendary Mod Explained

In Patch 62, the Rejuvenator Legendary Mod is receiving changes to how it functions, and the Rejuvenated Endurance perk card will play a larger role in boosting its effectiveness.
The base legendary mod will still provide HP and AP regeneration, but its strength will now scale further when paired with the Rejuvenated perk card. This means that staying Well Fed or Hydrated, and investing in the perk, will unlock higher tiers of regeneration than the legendary mod can provide on its own.
If you are a Ghoul, you still get the base HP and AP regeneration from the Rejuvenator legendary mod. However, because Ghouls don’t have Hunger or Thirst bars, they cannot equip the Rejuvenated perk card and therefore cannot gain the extra regeneration or buffs that come from it.
Rejuvenated Legendary Mod
Gain Health (HP) and Action Point Regeneration as You Fill Your Hunger and Thirst Meters.

Notes:
This effect only applies to members of your public or private team.
If you are a Ghoul, you and your teammates will still benefit from the base HP and AP regeneration buffs.
It does not affect non-teammates within the radius.
The effect is continuous, with no cooldown period, as long as teammates remain within the 50ft radius.
Effect stacks across teammates:
In a 4-person team where each member wears 5 pieces of Rejuvenator’s gear, the total stacking effect would be:
+20 AP regeneration per second
+20 HP regeneration per second

Rejuvenated Perk Card + Rejuvenated Legendary Mod
Ghouls cannot benefit from these buffs, as the Rejuvenated perk card cannot be equipped on Ghoul characters. Once you turn, you no longer have Hunger or Thirst bars, and the perk requires those to work.
The effects of the Rejuvenator legendary mod and the Rejuvenated perk card are structured into five tiers, each offering different levels of AP and HP regeneration. The buffs they provide to you and your teammates scale with certain condition checks, such as how full or empty your food and drink bars are and how many pieces of Rejuvenator you have equipped.
To use the information below, find the tier or benefits you’re interested in, then check which conditions need to be met for those benefits to apply.
Tier 5 (Max)
Player
AP Regeneration & HP Regeneration:+6/sec per Power Armor piece
1 piece = +6/sec
2 pieces = +12/sec
3 pieces = +18/sec
4 pieces = +24/sec
5 pieces = +30/sec
Team
Teammates in range receive the same regen (AP & HP):
1 piece = +6/sec
2 pieces = +12/sec
3 pieces = +18/sec
4 pieces = +24/sec
5 pieces = +30/sec
Conditions
Player is not a Ghoul
At least 1 Rejuvenator PA piece is equipped
Hunger is between Fully Fed and Well Fed (80% to 100%)
Thirst is between Fully Hydrated and Well Hydrated (80% to 100%)
Rejuvenated Rank 2 is equipped
Less than 10% Radiation on your HP bar
Tier 4 (Strong)
Player
AP Regeneration & HP Regeneration: +4/sec per Power Armor piece
1 piece = +4/sec
2 pieces = +8/sec
3 pieces = +12/sec
4 pieces = +16/sec
5 pieces = +20/sec
Team
